Job Summary:
Reports directly to the Home Health/Hospice Manager and is responsible for providing skilled physical therapy in the patient’s home, according to the Plan of Care as defined by the Physical Therapist. Works under the general/indirect supervision of the Physical Therapist.

Essential Duties and Responsibilities:
• Performs all patient treatments under the general or indirect supervision of a licensed Physical Therapist in accordance with state practice acts.
• Provides physical therapy services strictly adherence to the established Plan of Care developed by the Physical Therapist.
• Records accurate, timely clinical notes and objective observations following each patient session.
• Monitors the effects of treatment interventions and immediately reports improvements, unusual findings, patient decline, or safety concerns to the Physical Therapist and appropriate      Home Health/Hospice leadership.
• Assists the Physical Therapist in executing the Plan of Care, including but not limited to:
  o Implementing therapeutic exercises and functional training.
  o Providing patient and family education.
  o Delivering specialized interventions (e.g., stroke rehabilitation, cardiac rehabilitation, balancing techniques).
  o Training patients in ambulation with assistive devices.
  o Administering palliative care interventions (e.g., passive range of motion to prevent contractures).
• Participates actively in Clinical Record Reviews, quality assurance initiatives, and staff meetings.
• Attends mandatory in-service educational programs and maintains required continuing education units.
• Manages and coordinates the daily scheduling of assigned patient visits efficiently.
• Complies fully with FHP Home Health Policies and Procedures, including strict adherence to Infection Control guidelines.
• Promotes and utilizes approved pain management techniques to safely carry out the Plan of Care.
• Performs other related duties as assigned by leadership to support operations.

Job Specifications & Qualifications:
1. License is required. Issued by the Guam Board of Allied Health Examiners.
2.  Minimum of 2 years of experience in providing skilled physical therapy on an intermittent basis in the patient’s home and at times, for hospice patients in the acute care facility is preferred. The Physical Therapist will assess the Assistant’s skills if less than 2 years.
3. Must have the appropriate level of behavioral maturity and patience in handling patients.
4. Must have the diplomacy and tact in handling difficult and irate patients.
5. Very good interpersonal relationship skills and can work and relate well with co-employees, patients and customers.
6.  Outstanding communication skills and persuasiveness, specially the ability to communicate instructions and procedures for treatments, keeping the patients and giving them the desired level of comfort and confidence.
7.  Must have strong sensitivity to the needs and anxieties of patients and the ability to motivate patients to undergo treatments and/or procedures.
8. Strong ethics and a high level of personal and professional integrity.
9. Knowledgeable of federal and state laws and requirements relating to healthcare management.
10. Computer literate and very highly proficient in using MS office programs.
